If you are registering for the MVSC Recreational Soccer Program, and would like to request a full or partial scholarship, please complete the attached form and mail back to MVSC, along with any contribution you can make towards your child’s soccer season.

Please note that all scholarship families are expected to volunteer in some capacity, or submit the volunteer opt-out fee of $50, along with their partial registration contribution.  Your registration is not complete, and your child will not be placed on a team, until this Application Form is received.
